Passport applications are accepted during the following hours:

 

            Monday, Tuesday and Thursday:  10:00 a.m. to 2:00 p.m.

            Wednesday:                                   10:00 a.m. to 6:30 p.m.

           Passports are not accepted on Fridays

 

 

Fees are as follows and must be paid by check or money order made payable to

All minors must appear in person when applying for a passport.  All children under age 16 must have the consent of both parents in order to obtain a passport.  If one parent is unable to appear before the Accepting Agent, they must sign a notarize a consent form.  All children under the age of 16 must also submit a birth certificate, original, raised seal with both parents names listed on it.

 

 

An additional processing fee of $25.00 (per passport application) is paid to the Borough of Norwood and can be done by cash, check or money order.  If you are sending Express Mail an additional fee of $12.96 will be charged.

Requirements:

  • Completed application unsigned
  • Birth Certificates: an original certificate with a raised seal or a certified copy with a raised seal.  A hospital issued or baptismal certificate is not acceptable.  Birth Certificates issued by the Hudson County Clerk’s Office or Jersey City Registrar are no longer acceptable.  You must present a certified copy from the State of New Jersey.  You may request one at www.state.nj.us/health/vital/index.shtml .  All other Hudson County town issued birth certificates are acceptable.  All original documents will be returned to you with your passport
  • Previous Passport
  • Naturalization Certificate – original only.  This will be returned to you with your passport.
  • Driver’s License – if you are over 17 (under 17 you must bring a parent who has identifying documentation)
  • If you do not have a driver’s license you must have 3 forms of ID, one of which must contain your signature such as school/college ID, business/industrial/military ID, voter registration or credit card.
  • Two identical photographs – 2” x 2” – color.  Photos must have a white or off-white background.

 

Passport Renewal by Mail (Customer Submits)

You may qualify to use this form only if:

 

  1. The issue date of your last passport was less than 15 years ago;
  2. You were 16 or older when it was issued;
  3. Your name is the same or you can submit name-change documentation (marriage certificate or court order);
  4. You can submit this passport; and
  5. It is undamaged

 

Total fee for Passport Renewal by Mail is $110.00.  Follow all directions on Form DS-82.  This form is not accepted by our office.
